I, on behalf of myself, or on behalf of the minor described herein, acknowledge that participation in activities associated with MAI (the organization) is voluntary. I hereby acknowledge that participation in any athletic activities involves an inherent risk of physical injury. The undersigned does hereby release and forever discharge the organization, its members officially and individually, and its officers, agents, and employees of any and all claims, demands, rights, and causes of action of whatever kind or nature, arising from any injuries, damage to property, and the consequences thereof, including death, resulting from my participation in any way connected with events on behalf of the organization and I hereby assume all risk of involved participation in all the organization’s programs. I agree not to sue nor bring any type of lawsuit against any persons or entities mentioned above for any of the claims or liabilities that I have waived, released, or discharged herein; and I indemnify and hold harmless the persons or entities mentioned above from any claims made or liabilities assessed against them as a result of my actions or a result in my participation the organization’s programs. If I sustain injury or illness while participating in any of the organization’s activities, I hereby authorize any emergency first aid, medication, medical treatment or surgery deemed necessary by medical personnel. I also give permission for attending medical personnel to execute on my behalf, my permission forms, or other necessary medical documents and to act on my behalf if I am not immediately available to do so. I verify that I have no physical disabilities, impairments or chemical dependencies that inhibit my participation in sport activities. I do not know of any medical reason why I should not participate in the organization’s activities. I assume all medical expense responsibility for any injuries sustained in either voluntary or involuntary activities. I authorize my child/ren to travel to and from organizational activities in vehicles driven by staff and volunteers of the organization. These risks include and are not limited to any and all injuries to my child and any property damage associated with this travel. I hereby permit the taking and use of photographs, audio, and video to be used in any publication, website, social media account or advertising materials and release the organization from any liability. I also relinquish any right to examine and approve the completed materials prior to publication. I understand and agree that I have read and will comply with the policies contained in this document.
